OST513: Smoothing the Transition: Acute Care to
Post-Acute Care

PURPOSE OF ACTIVITY:

 

The purpose
of this activity is to inform and review evidence-based practice strategies
to help smooth the transition from acute care to post-acute care setting and
reduce hospital readmissions. 

COURSE DESCRIPTION:

 

The transition from
acute care to post acute care is difficulty for the ostomy patient.  Implementation of evidence-based practice
during this transition improves patients’ satisfaction, improves patients’
adjustment, reduces ER visits and reduces re-hospitalization.
